Please note that Travel Risk Assessment Committee (TRAC) defines "Short-Term" as less than 1 month and "Long-Term" as more than 1 month in duration.

Note on accessing existing saved forms: If you wish to access the form that you have already started, please click on the "LOGIN/REGISTER" link at the top right corner. You will be directed to login to your existing account and will have access to all the registration and application forms you have created previously.

Instructions
Please note that the "Short-Term" form is a brief one page form and the "Long-Term" form is a longer form with additional required items. 
  1. Click on the "Register Now" orApply Now button above next to the appropriate application for your trip. If you are not logged into SMC account, you will be directed to login using your SMC credentials.
  2. Once you are logged in, you will be directed to a page where you will be required to enter your travel itinerary (location and dates).
    • Please make sure to click the “Add to Itinerary” button to add each destination city in your itinerary. When you are finished with building your itinerary, please click on"Apply" button at the bottom of the page.
    • If you are visiting multiple cities, please enter your arrival and departure dates for ALL of your destination.
    • NOTE: If the country or city is not on the list of locations, please input the name of the destination CITY ONLY and click search. If you are still unable to add your location, do not waste your time. Instead, please contact SMCTravel@stmarys-ca.edu so that we could add your location in the system.
  3. Answer rest of the questions on the form or complete the rest of the required items.
